Be more engaged in your own personal well-being than ever before with the Care Plan! The Care Plan summarizes all your vital health data and connects you with the providers of your choice, by putting you all on the same platform.

Providers can add or edit all the essential information (such as diagnoses, goals, future check-ups, tasks, medication and notes) on your profile, which greatly improves coordination and optimizes treatment.

To get to the Care Plan, first login to your Qure4u profile, and then click ‘Care Plan’ at the top of the page or on the dashboard.
